Entries are invited from anyone working in the diabetes arena. Whatever the area of patient care, if your initiative or programme relates to diabetes in any way, you should enter it. Judges are keen to review a wide spectrum of entries and recognise the innovative practice that we know is out there.
We welcome initiatives and programmes that were launched between January 2010 and December 2015.
Take a look at the categories to decide which one is most appropriate for your entry. Please note that an initiative can be entered in one category only. However an entrant can submit multiple initiatives into the appropriate categories.
Entries for certain categories are decided by nomination. Submit your choice for who should win the Diabetes Healthcare Professional of the Year, People's Award or Outstanding Educator in Diabetes.
If you are proud of the good work you are doing, help others by sharing your success and gain nationwide recognition for what you do.
The entry process is simple so entering will not take up too much time.

Each entry will be reviewed by a panel of high-level diabetes experts, chaired by Simon O'Neill, Director of Health Intelligence and Professional Liaison, Diabetes UK, Dr Paru King, Consultant Physician, Derby Hospitals and Dr Sheridan Waldron, Education Lead, Children and Young People's Diabetes Network.
Finalists will be notified and then invited to hear the winners announced on Thursday October 13, 2016, at the QiC Diabetes Awards presentation at Sanofi's headquarters in Guildford.
At the Awards presentation, the top three finalists in each category, as well as the winners of the Judges' Special Award, Diabetes Healthcare Professional of the Year Award, People's Award and Outstanding Educator in Diabetes, will be presented with a certificate or a trophy.
Finalists will be featured on QiC Connect, an online knowledge hub packed full of examples of healthcare initiatives that have been successfully implemented by health providers and others.
In this, the sixth year of QiC Diabetes, we welcome initiatives and programmes that were launched between January 2010 and December 2015.

PMGroup will keep all information provided confidential and all judges will sign a confidentiality agreement prior to appointment. Entrants may mark certain sensitive parts of their entry 'not for publication', provided such restrictions are not used unreasonably.
