How to enter


Entries are invited from anyone working in the diabetes arena. Whatever the area of patient care, if your initiative or programme relates to diabetes in any way, you should enter it. Judges are keen to review a wide spectrum of entries from across the UK and recognise the good practice that we know is out there.

We welcome initiatives and programmes that were launched between January 2008 and December 2011.

Take a look at the categories to decide which category is most appropriate for your entry. You can enter the same project in a maximum of two categories. Organisations can enter separate projects in multiple categories.

Entries for certain categories are decided by nomination. Submit your choice for who should win the People's Award or Outstanding Educator in Diabetes.

Why should I enter?
If you are proud of the good work you are doing, help others by sharing your success and gain nationwide recognition for what you do.

How do I enter?
The entry process is simple so entering will not take up too much time.

Step one: Download an entry form to your computer and complete all the sections. Once complete, come back to the QiC website to upload and submit your entry.

Step two: Submit your entry and supplementary material online. You can also email your entry form, and supporting documents, to entry@qualityincare.org

What happens next?
Each initiative will be reviewed by a panel of high-level diabetes experts, chaired by Anna Morton, director, NHS Diabetes, and Simon O'Neill, director of care, information and advocacy at Diabetes UK, and the finalists will be selected.

Finalists will be notified and then invited to hear the winners announced on October 18, 2012, at the QiC Diabetes Awards presentation at Sanofi's headquarters in Guildford.

At the Awards presentation, all of the top three finalists in each category as well as the winner of the People's Award will be presented with an award.

Finalists will be featured in QiC Connect, an online knowledge hub packd full of examples of healthcare initiatives that have been successfully implementedn by health providers and others up and down the UK.

Eligibility
In this, the second year of QiC Diabetes, we welcome initiatives and programmes that were launched between January 2008 and December 2011.

Entry fee
Entry is free to anyone working for a UK health service or patient group. The fee for entries from the private sector and commercial organisations is £150 + VAT per entry, to be paid on receipt of an invoice which will be sent to you once we've received your submission. Payment may be made by credit or debit card, BACS transfer or PayPal.

Confidentiality
PMGroup will keep all information provided confidential and all judges will sign a confidentiality agreement prior to appointment. Entrants may mark certain sensitive parts of their entry 'not for publication', provided such restrictions are not used unreasonably.
