Nomination for: QiC Diabetes People's Award
What is the People's Award?
This is a special award, supported by Diabetes UK, which aims to recognise someone very special who cares for/has cared for people with diabetes. The award will be presented to someone that has made a real difference to the lives of people with diabetes and goes beyond the call of duty with regards to what is required in their day job.
You may nominate a carer, consultant, DSN, nurse, family member – anyone that you think deserves this recognition.
You may not nominate yourself.
Nomination process
All nominations received by the entry deadline will be uploaded to the Quality in Care Diabetes website for the online voting process which will be open to the public vote.
Entry for this category will close on May 30 and the nominations posted on the Quality in Care Diabetes website for the public to vote on from June 12 to July 26.
The winner will be announced online following the QiC Diabetes awards ceremony in October.
How to nominate a person for the People's Award?
Please provide:
• Brief explanation for why your nomination deserves to be recognised - please demonstrate results that have quality and/or efficiency benefits for people living with diabetes • Two endorsements, ideally one from a health professional and one from a patient • Any supporting materials, including links to blogs and websites • If available, a current photograph of the person you are nominating (JPG, GIF or PNG)
