What is QiC Diabetes?
QiC Diabetes is a multi-tiered programme of activity that will run through 2011-2012 and ultimately aims to deliver improved patient outcomes through the shared goals of improving awareness, prevention and detection of diabetes, together with improved access to treatment.

Who can enter?
The programme is open to anyone working in the diabetes community; in a UK health service; for a patient group; in the voluntary sector; industry; in a consultancy; or people with diabetes.

What work is eligible?
All work entered must be carried out in the UK.
Except where indicated, programmes and initiatives should relate to the period January 2008 to December 2011.
Organisations, companies or individuals may submit entries on behalf of themselves or others and may enter the same project in a maximum of two categories. Organisations can enter separate projects in multiple categories.

How do I enter?
Download the entry template from our 'How to enter' page and provide information under the headings shown. All entries should follow the template provided. Any entries received not using the template will be disqualified. Submit your entry using our online entry process, or by email to entry@qualityincare.org. When is the entry deadline?
The entry deadline is Friday May 25, 2012.

Can I enter in more than one category?
Organisations may enter the same project in a maximum of two categories and separate projects in multiple categories.

What's the entry process?
Simply complete the entry template and send to entry@qualityincare.org with any supplementary materials, or by post to:

Is there an entry fee?
Entry is free to anyone working for a UK health service or patient group. Fee for entries from the private sector and commercial organisations is £150 +VAT, to be paid on receipt of an invoice which will be sent to you once we've received your submission.

How do you deal with confidential information?
The organisers reserve the right to publish information provided in your summary for subsequent publication.
Descriptions of submissions from finalists will be published on QiC Connect, a web-based resource providing practical advice and ideas on how providers can develop or improve service provision and patient outcomes..
Entrants may mark certain sensitive parts of their entry 'not for publication', provided such restrictions are not used unreasonably.

Who are the judges?
We are delighted to welcome back Anna Morton, director, NHS Diabetes, and Simon O'Neill, director of care, infomration and advocacy, Diabetes UK, as our Chairs of judges. Together with NHS Diabetes and Diabetes UK we are inviting commissioners, clinicians, third sector representatives and leading industry professionals to join the judging panel to review entries in the QiC Diabetes programme. When is the shortlist announced?
Shortlisted entries will be announced after the judging day on www.qualityincare.org. Judging day will be held on September 26, 2012.

When are the winners announced?
Winners will be announced at the Awards ceremony on October 18, 2012 at Sanofi's headquarters in Guildford.

Is there a prize?
The top three entries in each category and the winner of the People's Award will all receive trophies.

How do I book tickets for the Awards ceremony?
Tickets are available for the Awards ceremony at Sanofi's headquarters, One Onslow Street, Guildford, Surrey, GU1 4YS, on Thursday October 18, 2012. Email lbellamy@qualityincare.org for further details. 

How will you publish the results?
Following presentation of the Awards, the finalists and winners will be announced here on www.qualityincare.org. Details of the commended initiatives will be featured on QiC Connect, an online knowledge hub packed full of examples of healthcare initiatives that have been successfully implemented by health providers and others up and down the UK.
