What is QiC Diabetes?
QiC Diabetes is a multi-tiered programme of activity that will run through 2013-2014 and ultimately aims to deliver improved patient outcomes through the shared goals of improving awareness, prevention and detection of diabetes, together with improved access to treatment.

Who can enter?
The programme is open to anyone working in the diabetes community; in a health service; for a patient group; in the voluntary sector; industry; in a consultancy; or people with diabetes. Entries are not limited to the UK.
What work is eligible?
Except where indicated, programmes and initiatives should relate to the period January 2008 to December 2012. Please refer to category description for criteria details.
Please note that an initiative can be entered in one category only. However an entrant can submit multiple initiatives into the appropriate categories.
Please note that judges retain the right to reassign entries to an alternative category than the one to which it has been submitted, if the judges feel it would be more advantageous for the entry.
Particular credit will be given to projects which demonstrate results having quality and/or efficiency benefits which could be replicated elsewhere, thereby having a greater impact than their initial focus.

How do I enter?

When is the entry deadline?
The entry deadline is 18:00hrs, Friday May 10, 2013.
How can I be sure that you have received my entry?
When you submit your award by email, you will receive an email confirming receipt of your entry. If you have not received a confirmation email within 48 hours of submitting your entry, please call or e-mail Emma Clear on +44(0)1372 414200 or eclear@qualityincare.org
Can I enter in more than one category?
Organisations may enter the same project in a maximum of one category and separate projects in multiple categories.
Can I change or withdraw my entry after submission?
You can withdraw your entry at any time. Changes are not permitted to your entry after the submission deadline has passed.
Can I resubmit a previous entry?
Entries may be resubmitted only if there has been a significant new aspect to the programme and evidence of additional outcomes and results which are now available and were not at previous time of submission.
What's the entry process?
Completed forms should be emailed to entry@qualityincare.org.

How do you deal with confidential information?
The organisers reserve the right to publish information provided in your summary for subsequent publication.
Descriptions of submissions from finalists will be published on QiC Connect, a web-based resource providing practical advice and ideas on how providers can develop or improve service provision and patient outcomes.
Entrants may mark certain sensitive parts of their entry 'not for publication', provided such restrictions are not used unreasonably.

Who are the judges?
We are delighted to welcome back Simon O'Neill, Director of Clinical Intelligence and Professional Liaison, DIabetes UK, as one of our chairs of judges.
Together with NHS Diabetes and Diabetes UK we are inviting commissioners, clinicians, third sector representatives and leading industry professionals to join the judging panel to review entries in the QiC Diabetes programme. Meet the judges.
Can you explain how the entries are reviewed?

When is the shortlist announced?
Shortlisted entries will be announced on www.qualityincare.org after the judging day. Judging day will be held on July 18, 2013.
When are the winners announced?
Finalists will be invited to the Awards ceremony where the winners will be announced on Thursday October 10, 2013 at Sanofi's headquarters in Guildford.
Is there a prize?
The top three entries in each category will all receive either a certificate or a trophy.
How will you publish the results?
Following presentation of the Awards, the finalists and winners will be announced here on www.qualityincare.org. Details of the commended initiatives will be featured on QiC Connect, an online knowledge hub packed full of examples of healthcare initiatives that have been successfully implemented by health providers and others up and down the UK.
Is it possible to receive the judges’ comments on my entry if I am not successful?
Due to the historic volume of entries, the QiC Diabetes judges are unable to offer individual comments to entrants.
How will the materials I submit be used?
If your entry is shortlisted, the Programme organisers may like to reproduce a summary of your entry, photographs and any other supporting materials in the winners’ booklet or within other materials they produce to promote the Awards (eg press materials or on the website). On each award application form, there is a section for you to confirm you are happy for your materials to be used in this way. The Programme organisers will not use any materials if you do not give express permission.
